Thriving as a doctor: Why I still love my medical practice amidst burnout fears

Gerald P. Corcoran, MD

July 4, 2024

I have always felt that I was in the mainstream of medical thought and actions. Recently, however, I have been subjected to a barrage of articles on physician burnout, depression, and even suicide! I am happy to be working in my medical practice. What is wrong with me?

Like everyone, it is difficult to drag myself out of a nice, warm bed each morning. But as I begin to dress for …

Warehouses for the elderly?

May 30, 2022

Bringing in the mail recently, I was pleased to see a thick 5-by-7-inch booklet among the catalogs and bills. The anticipation of reading a few Reader’s Digest short stories and “Humor in Uniform” made the evening seem pleasurable. But when I opened the 244-page Reader’s Digest-sized booklet, I found the “Senior’s Blue Book for Greater Boston.” Here was page after page of assisted living communities, retirement communities, skilled nursing facilities, …

Dislocations: an exception to the rule

Gerald P. Corcoran, MD
Conditions
May 4, 2022

There are not many situations in general medicine where a doctor can affect an instant cure, and the patient will call them a “miracle worker.” Most patient visits end with a prescription or two or some instructions along with the admonition, “You’ll feel better in a few days.”

One exception to this rule: dislocations.

A simple dislocation happens when the end of a bone is pulled out of its socket (or niche) …

A primary care physician’s gut feelings

Gerald P. Corcoran, MD

March 22, 2022

To spend an entire college year (two semesters) studying epistemology seemed like a colossal waste of time when I was 19. We had no choice; we had to take it, and I got a pretty good mark, as I recall. It took a half year to find out what it was about, but it seemed to concern itself with how we obtain knowledge. There is inductive reasoning and deductive reasoning …

Glowing and beaming represent physical evidence of joy in life

Glowing and beaming.

Applying these terms to people has become somewhat dated, but to students of human nature, and especially doctors who meet people under all sorts of unusual circumstances, they still apply.

A former colleague of mine, who worked in general practice in Kansas in his early medical years, went on to an OB/GYN residency and practiced obstetrics until his retirement. He swore that he could tell when a woman was …

The toxic doctor-insurer relationship

Gerald P. Corcoran, MD

November 6, 2021

My introduction to health insurance companies was very abrupt and simple.

I received my medical training in the U.S. Navy and served three years after that. Then, I was finally discharged. I had treated thousands of patients as a Navy doctor, but I had never charged one, nor did I even know how to charge for services.

My partner in the civilian practice I joined made the transition easy: “Sign this contract …

The telehealth problem: full pay, but less value

Gerald P. Corcoran, MD
Physician
August 27, 2021

The regulation allowing full reimbursement for telehealth visits in the setting of the pandemic was a wise and compassionate decision. As more doctors participated, many thousands — likely millions — of sick and susceptible elderly were able to access needed care without exposing themselves to COVID-19. It was a double-edged sword, however, and we are now seeing the downside of continuing such a policy beyond pandemic concerns. The problem: Everybody …

Why I became a doctor: reflections on a calling

Gerald P. Corcoran, MD
Physician
June 24, 2021

The landscape of medical practice has changed dramatically over the many decades I have been in practice. Most of these changes are intensely discouraging and depressing and are negatively affecting the physicians of today. The government, hospitals, networks, even our own professional organizations are working to establish and enforce new regulations — new hoops for doctors to jump through, mainly in the furtherance of their profit and their power over …

Lessons about love from the too-often overlooked among us

Gerald P. Corcoran, MD

February 25, 2021

“Last question,” the woman from the insurance company said.

Good, I thought; I was eager to get back to seeing patients.

“How long have you been taking care of Sarah?”

“I’m not sure,” I replied, “How old is she?”

“She’s 34.”

“Then I have been taking care of her for 35 years.”

“Doctor, I’m not sure you heard me correctly; she is only 34.”

“I heard you perfectly,” I retorted, “She has been my patient for 35 …

It’s time to revisit the 4 R’s of the doctor-patient relationship

Gerald P. Corcoran, MD

January 7, 2021

“We have met the enemy in medical education, and he is us!”

My paraphrase of the “philosophy of Pogo” is pertinent to today’s crop of graduating medical students. We have inserted them into the most toxic environment for learning medicine, ever.

Dr. Michael Halberstam once stated that the most powerful treatment the ER had to offer (after CPR, of course) was “the sight of your own doctor at the bedside.”  Yet look …
